This study looks at patients having a type of stroke called an *acute ischemic stroke* caused by a blockage in a large blood vessel. The focus is on patients who arrive at the hospital more than 24 hours after their last known well time. Doctors use special treatments like **endovascular therapy (EVT)**, which involves a small tube to remove the blockage, or the best available medicines called **best medical management (BMM)**.
Doctors want to see if EVT is better than BMM for patients who arrive late. They will gather lots of information about the patients, including their medical history and the test results that show where the blockage is. Special imaging tests like **magnetic resonance angiography (MRA)** and **computed tomography angiography (CTA)** help find the blocked vessel. This study will help doctors understand the best way to treat these strokes in the future.
- Study includes patients from Jan. 1, 2018, to Jan. 1, 2028.
- Involves late arrival after 24 hours from last known well.
- Patients can receive EVT or best medical treatment.
